Appointments

Make an appointment online

There is now a new way to contact us online called PATCHS.

Appointments System

If you would like to request an appointment you can complete an online consultation via PATCHs. You will need to register by providing and email address and choosing a password. Then complete a short form so that your GP can better understand and address your health needs.

Alternatively you can still telephone or visit the practice. The reception desk is open Monday to Friday 08:30 to 19:00 (phones manned up to 18:30).

Our phone lines tend to be very busy first thing in the morning so it would be helpful if you could telephone for routine appointments after 10:00.

My Planned Care

My Planned Care gives you advice and support while you wait and helps you to prepare for your hospital consultation, treatment, or surgery. This includes giving you information about waiting times at your hospital and other supporting and local services while you wait.

This site is updated weekly and can be viewed by anyone, which means you, your family or carer as well as your NHS team can all see the latest information.

Your hospital team will be in touch with you as soon as they can. If you are looking for an update, please check this website before contacting your hospital or GP.

How to See a Doctor, Nurse or Nurse Practitioner

The majority of consultations are by appointment.

You can book for an appointment during the times shown in the table below.

Routine Appointments

We have introduced PATCHS online health questionnaire tool to improve access for patients. You can now complete a form to get help with your healthcare needs instead of visiting or telephoning the practice. You will be contacted within 24 hours (one working day) of receipt of your PATCHS request.

You can still telephone the practice on 020 8866 0075 or visit the practice. Our receptionist will ask some questions to help get you the right help you need. This will involve you giving details of the problem to help identify the most appropriate pathway. All telephone calls are recorded for monitoring and training purposes

Routine appointments are usually booked two or more days in advance – the earlier the booking, the better the chance of getting a particular time or particular doctor. This also means any test results can be retrieved in advance and generally enables greater efficiency.

Please note that our receptionists may ask for details of your condition to assist medical staff – all such information is in complete confidence.

Home Visits

These are for patients who are too ill to come to the surgery. If you need a home visit please phone 020 8866 0075 between 08:45 and 11:00.

We would request that, apart from the genuinely housebound, all other patients attend the surgery rather than request a home visit because of the extra time home visiting takes. On average four to five patients can be seen in surgery in the time it takes to do a single house call.

In addition, the care that can be offered in your own home is not as good as in the surgery because of the lack of examination facilities and equipment means that you may not receive as good a service as the doctor may be able to offer if you came to the surgery.

Non-urgent advice: Notice

Please note that the doctor may telephone you rather than visit you if this is medically appropriate. Ultimately it is the doctor’s responsibility to decide whether or not a visit is appropriate for a particular set of circumstances.

Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, as this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters may be removed from the list.

Late For Your Appointment

Please attend your appointment on time. If you are late you may not be seen and you will not be able to rearrange your appointment until the next working day – except in the event of a medical emergency that requires immediate attention.

Extended Practice Opening Hours

Appointments are now available outside standard GP Practice opening hours. These appointments will be divided between three HUB sites across Hillingdon.
